В обзоре рассматриваются патогенетические механизмы, факторы риска и клинические проявления данной патологии, ее распространенность. Особое внимание уделяется проблеме сочетания реноваскулярной артериальной гипертензии с дисфункцией почек и с проявлениями сердечной недостаточности, что наиболее характерно для двусторонних гемодинамически значимых стенозов или резко выраженного стеноза единственной функционирующей почки, и позволяет сформулировать представление об атеросклеротической реноваскулярной болезни. В обзоре содержатся современные представления о диагностике этого заболевания и тактике ведения больных с данной патологией. Особое внимание уделяется показаниям для реваскуляризации и способам восстановления кровотока по почечным артериям. В обзоре подробно обсуждаются проблемы ведения больных после интервенционного лечения и прогноз пациентов с атеросклеротической реноваскулярной болезнью.</p></abstract><trans-abstract xml:lang="en"><p>The literature review is devoted to one of the most common secondary forms of arterial hypertension – renovascular hypertension of atherosclerotic genesis. The review examines the prevalence of this problem, pathogenetic mechanisms, risk factors and clinical manifestations. Special attention should be paid to the problem of combining renovascular arterial hypertension with renal dysfunction and with manifestations of heart failure, which is most typical for bilateral hemodynamically significant stenoses or significant stenosis of a single functioning kidney, and allows us to formulate an idea of atherosclerotic renovascular disease. The review contains modern ideas about the diagnosis of this disease and the tactics of managing patients with this pathology. Special attention should be paid to indications for revascularization and ways to restore blood flow through the renal arteries.
